Wellbeing Support Service for Older Adults

The Wellbeing Support Service for Older Adults provides access to free in-home psychological services for residents residing in Aged Care Facilities within the South East Melbourne Region.

What is the Wellbeing Support Service for Older Adults?

The Wellbeing Support Service for Older Adults is a Commonwealth Government Service, through the South Eastern Melbourne Primary Health Network (SEMPHN), to provide psychological services to people living in Residential Aged Care Facilities in the South East Melbourne Region.

The Wellbeing Support Service for Older Adults is designed to meet the needs of residents with low to mild mental health concerns, or those who may feel at risk of developing mental health concerns, and would benefit from individual counselling.

The service is designed to be highly flexible in their delivery to match an individual’s needs:

  • Individual counselling 1:1 sessions provided by allied health professionals.
  • Group sessions targeted to meet the specific needs of residents residing in Residential Aged Care.
  • Family support and liaison.
  • Care-coordination and liaison.
  • Flexibility with service delivery (such as via videolink, telephone, and in person) to meet the needs of both the individual and Residential Aged Care facility.

Who Can Access Star Health Wellbeing Support Services?

Star Health Wellbeing Support Services are for people who:

  • live in Residential Aged Care within the South East Melbourne Region (Bayside, Cardinia, Casey, Dandenong, Frankston, Glen Eira, Kingston, Mornington Peninsula, Port Philip and Stonnington).
  • have mild to low mental health concerns or feel at risk of developing mental health concerns.
  • would benefit from mental health support via 1:1 counselling or group sessions from an allied health professional.

How can I access the Wellbeing Support Service for Older Adults?

Referrals to the Wellbeing Support Service can be made by residents, family members, Residential Aged Care facility staff or GPs.

A referral can be submitted by completing the Wellbeing Support Service for Older Adults Referral Form or by directly contacting the referral contact numbers listed below.
